Getting Started
Initial Diagnostic Evaluation — $495 (60 minutes)
A comprehensive psychiatric assessment including history, diagnostic impression, treatment planning, and medication options as appropriate. Brief skills-based/solution-focused therapy may be included, along with measurement-based screening when clinically indicated. This is to learn about your current symptoms, your past, and your goals for treatment. This may involve, with your permission, contacting providers or family members who know you well and who may provide a richer perspective on your concerns. After the assessment, we will review evidence-based treatments and possible referrals to other providers to promote wellness.

Form Completion Requests
Additional fees for phone calls, letters, emails, reports, etc. will be determined on a time for service basis for established patients. From completion requests are considered adminstrative, non clinical tasks, requiring significant time outside of clinical duties, and is considered a patient out-of pocket expense.
- FMLA, school/work forms, letter, emails, reports: $200/hour, billed in 15 min increments (minimum 15 min)
- Care Coordination (PCP/specialists/therapist call): $150/15 min
- Records review beyond what is included in the evaluation: $150/ 30 min
We are unable to perform the following:
- Group, family, or couple therapy
- Disability evaluations
- Forensic evaluations
- Court-ordered treatments
- Assessments for child custody, work, weapon ownership, or emotional support animals.
